WHAT SHOULD I BRING WITH ME TO MY FIRST APPOINTMENT?
Come 30 minutes early for the first appointment to fill out paperwork. Bring an updated medication list and insurance card. If you wish, you can print the following forms* to complete at your convenience. Remember to bring them with you to your visit.
*Adobe Reader is needed to open the forms for printing.
DO YOU TAKE URGENT CARE OR SAME-DAY APPOINTMENTS?
Call the office as early in the day as possible and our staff will do their best to get you an appointment that day.
DO YOU HAVE A CANCELLATION POLICY ?
24 hours notice of a cancellation is appreciated. There will be a fee of $25 for missed appointments.
WHAT INSURANCE PLANS DO YOU ACCEPT?
We accept a wide variety of insurance plans. Please contact us to inquire about which plans we accept. Co-pays, deductibles and Co-insurances are due at time of service.
